Understanding Antisocial Personality Disorder
Antisocial Personality Disorder (ASPD) is characterized by a pervasive pattern of disregard for, and violation of, the rights of others – including deceitfulness, impulsivity, aggressiveness, reckless disregard for safety, consistent irresponsibility, and lack of remorse. ASPD exists on a spectrum, from mild presentations involving interpersonal difficulties and rule violations, to severe presentations involving significant harm to others. Many people with ASPD have significant trauma histories – particularly childhood trauma and adverse experiences – that have shaped the personality patterns. Not everyone with ASPD lacks the capacity for change or meaningful therapeutic engagement. Genuine motivation to address the costs of ASPD patterns – to relationships, to employment, to freedom – can provide meaningful clinical traction.

About Antisocial Personality Disorder Therapy
Why seek therapy?
People with ASPD most commonly seek therapy following external pressure – legal requirements, relationship ultimatums, or employment consequences. Internal motivation may be limited initially, and the therapeutic work often begins with building genuine rapport and exploring what the person actually wants their life to look like. Where genuine motivation for change is present, therapy addresses the interpersonal patterns, the impulsivity, and the underlying emotional and relational deficits that characterize ASPD.
How therapy helps
Therapy for ASPD uses a pragmatic, non-moralistic approach that focuses on the practical costs of ASPD patterns and the genuine alternative ways of meeting needs and managing situations. CBT addresses impulsivity and decision-making patterns. Mentalization-based approaches develop the capacity to understand one’s own and others’ mental states – a capacity that is often underdeveloped in ASPD. Motivational approaches build genuine reasons for change that are grounded in the person’s own values and goals.
Benefits of Antisocial Personality Disorder Therapy
Non-Judgmental, Expert Engagement
Effective ASPD therapy does not begin with moral judgment or demands for remorse. It begins with genuine engagement with the person’s actual situation, goals, and the practical costs of their current patterns.
Addressing the Costs Practically
ASPD patterns carry real costs – to relationships, employment, freedom, and quality of life. Therapy addresses the practical alternatives to these patterns in ways that serve the person’s genuine interests.
Building Genuine Rapport
The therapeutic relationship is the primary vehicle of change in ASPD therapy. Building a genuine, non-exploitative therapeutic relationship – often a novel experience – is itself therapeutically important.
ASPD deserves expert, non-judgmental therapeutic engagement. That is what we provide.

Our Approach to Antisocial Personality Disorder Therapy
ASPD therapy at Empire begins with building a genuine therapeutic relationship – which is itself therapeutically significant for a person whose experience of relationships has typically been transactional or adversarial.
We take a pragmatic rather than moralistic approach – focusing on the practical costs of ASPD patterns and exploring what the person actually wants their life to look like.
Mentalization-based approaches develop the capacity to understand one’s own and others’ mental states – a foundation that supports more effective and less costly interpersonal navigation.
Motivational approaches build genuine internal motivation for change that is grounded in the person’s own values and goals – because change that is externally imposed rarely endures.

Common Questions About Antisocial Personality Disorder Therapy
I have been told ASPD is untreatable. Is that true?
No. While ASPD is more challenging to treat than some conditions, evidence-based therapy can produce meaningful change – particularly where there is genuine motivation to address the costs of current patterns. The prognosis is significantly better in adults who are motivated to engage.
I am seeking therapy because of legal or relationship pressure. Is that a valid reason?
Yes. External pressure is often the initial driver of therapy engagement in ASPD. The therapeutic work involves developing genuine, internal motivation alongside the external requirements.
Is a referral required?
No. You can book directly online or by calling (905) 962-2220.
History of Antisocial Personality Disorder Treatment
Evolution of Treatment
Personality disorders were long considered untreatable or extremely difficult to treat, and the stigma associated with them contributed to inadequate care. The development of Dialectical Behaviour Therapy by Marsha Linehan in the late 1980s was a watershed moment – providing the first strongly evidence-based treatment specifically developed for personality disorder presentations, particularly BPD. Subsequent decades have seen significant advances in understanding and treating personality disorders.
A Modern Approach in Canada
Current best practice in Canada uses evidence-based psychological therapies – DBT, Schema Therapy, Mentalization-Based Treatment, Transference-Focused Psychotherapy – as the primary treatments for personality disorders. Medication may address specific symptoms but is not a primary treatment. The field increasingly emphasizes the role of early relational experiences in personality disorder development, and the importance of a strong, stable therapeutic relationship as the primary vehicle of change.
Genuine motivation to change deserves genuine therapeutic support. We provide that.
