Understanding Histrionic Personality Disorder
Histrionic Personality Disorder (HPD) is characterized by a pervasive pattern of excessive emotionality and attention-seeking behaviour, including discomfort when not the centre of attention, interactions characterized by inappropriate sexual seductiveness or provocation, rapidly shifting and shallow emotional expression, use of physical appearance to draw attention to self, impressionistic and lacking in detail speech, theatrical and exaggerated expression of emotion, suggestibility, and a tendency to consider relationships as more intimate than they actually are. HPD typically develops in the context of early experiences in which emotional needs were inconsistently met and in which attention and approval were highly valued. The attention-seeking and dramatic emotionality that characterize HPD are attempts to meet genuine needs for recognition, connection, and worth that have not found more effective pathways.

About Histrionic Personality Disorder Therapy
Why seek therapy?
People with HPD often seek therapy when relationships – which they invest in intensely – consistently fail to provide the depth of connection they seek; when patterns of behaviour have been pointed out by others as problematic; or when the emotional intensity and drama of their internal life has become genuinely distressing. Many do not recognize the HPD pattern in themselves initially.
How therapy helps
Therapy for HPD uses CBT and Schema Therapy as the primary approaches – addressing the core beliefs about self (that worth depends on being attractive, exciting, and the centre of attention) and others (that emotional responsiveness requires dramatic expression and attention-seeking) that maintain HPD patterns. Skills work builds more sustainable and genuine ways of meeting needs for recognition, connection, and worth. The therapeutic relationship provides a consistent, genuine connection that is not contingent on dramatic presentation.
Benefits of Histrionic Personality Disorder Therapy
Understanding the Underlying Needs
HPD patterns are attempts to meet genuine needs for recognition, connection, and worth. Therapy helps identify and address those underlying needs through more effective and sustainable pathways.
Building Genuine Depth
HPD often produces relationships that feel superficial despite significant investment. Therapy builds the capacity for genuine intimacy and depth – the real connection that HPD patterns have been seeking but not achieving.
Sustainable Self-Worth
HPD involves a self-worth that depends heavily on attention and admiration. Therapy builds a more stable, internally sourced sense of self-worth that does not depend on being the centre of attention.
The connection you are seeking is possible – through a different path than the one HPD has been taking you on.

Our Approach to Histrionic Personality Disorder Therapy
HPD therapy at Empire begins with genuine warmth and engagement – recognizing the real needs for connection and recognition that HPD patterns are attempting to meet, without reinforcing the dramatic or attention-seeking dimensions of the presentation.
CBT addresses the core beliefs about self-worth and the assumptions about how needs must be met that maintain HPD patterns, developing more accurate and sustainable alternatives.
Schema Therapy addresses the early maladaptive schemas – particularly approval-seeking and emotional deprivation – that underlie HPD, tracing their origins and working to meet those underlying needs in more effective ways.
Skills work builds more genuine, sustainable ways of connecting with others, expressing emotion, and meeting needs for recognition – so that relationships become deeper and more satisfying.

Common Questions About Histrionic Personality Disorder Therapy
I was told I have HPD. I do not think I am dramatic – I just feel things intensely. How do I make sense of this?
This is a common experience. HPD involves genuine emotional intensity, not manufactured drama. The therapeutic work helps understand the pattern from the inside – and develop more effective ways of communicating and connecting that produce the genuine responses being sought.
My relationships always feel superficial even though I invest in them intensely. Is that related to HPD?
The experience of intense investment in relationships that nevertheless feel shallow or unrequited is one of the most painful features of HPD – and one of the most important things therapy addresses.
Is a referral required?

History of Histrionic Personality Disorder Treatment
Evolution of Treatment
Personality disorders were long considered untreatable or extremely difficult to treat, and the stigma associated with them contributed to inadequate care. The development of Dialectical Behaviour Therapy by Marsha Linehan in the late 1980s was a watershed moment – providing the first strongly evidence-based treatment specifically developed for personality disorder presentations, particularly BPD. Subsequent decades have seen significant advances in understanding and treating personality disorders.
A Modern Approach in Canada
Current best practice in Canada uses evidence-based psychological therapies – DBT, Schema Therapy, Mentalization-Based Treatment, Transference-Focused Psychotherapy – as the primary treatments for personality disorders. Medication may address specific symptoms but is not a primary treatment. The field increasingly emphasizes the role of early relational experiences in personality disorder development, and the importance of a strong, stable therapeutic relationship as the primary vehicle of change.
Genuine connection is possible. Therapy helps you find a more effective path to it.
